Enrollment materials are also available at Calvert County Libraries, Calvert Memorial Hospital Case Management Office, Calvert County Health Department, Calvert County Department of Social Services, or by calling (443) 404-5761.

WHO IS ELIGIBLE?

Qualifying individuals must:

1. Be a Calvert County Resident

2. Be age 19 to 64 years old

3. Earn between $12,672 and $21,660 a year (higher for families)

4. Be without health insurance

5. Be a U.S. citizen or Documented permanent Resident

6. Complete and submit an Application with supporting documents

7. Be willing to comply with program requirements as defined in the Program Guidelines.
