Program Guidelines

General Requirements:

1.	Must earn between $12,672 and $21,660 a year (higher for families)

2.	Must be without health insurance

3.	Be age 19 to 64 years old

4.	Be a U.S. citizen or Documented permanent Resident

4.	Complete and submit an Application with supporting documents

5.	Be willing to comply with program requirements
