Program Guidelines

General Requirements:

1.	Must earn between $12,168 and $20,800 a year (higher for families)

2.	Must be without health insurance

3.	Be age 19 to 64 years old

4.	Complete and submit an Application with supporting documents

5.	Be willing to comply with program requirements
